                                  We suggest 20 minutes prior to your service time.
    A representative from Arena Food Service is happy to assist in planning your event.
    By submitting this requisition form, you acknowledge receipt, understanding and agree to the policies, rules and conditions of Governors State University and Arena Food Service, Inc., implied or written, in relation to hosting events at GSU.  Event guidelines can be found within the Catering Guide.  The submittor also agrees that he/ she is authorized to place such an order and is the responsible party to ensure payment.
